We are a boutique consulting firm offering managed care consulting, mentorship to physicians and pharmacists and thought leadership. Our journey started in 2021, when Founder Dr. Emran Rouf started reviewing CVs of physicians and other healthcare professionals, and preparing them for mock job interviews. The work also included the review of physician job contracts and coaching services to physicians as they transition into new jobs.
After two years of career coaching, Dr. Rouf started a niche consulting service for small health plans and community health systems. Some of his work included workforce management and forecasting for utilization management, pharmacy services, remote patient monitoring tools for cardiovascular conditions.
In 2025, Lena Miller joined the team to offer a core set of pharmacy services and continue to support the mentoring role for pharmacists and other allied health professionals.
Dr. Emran Rouf is a seasoned physician executive with 20+ years of healthcare experience. He has had progressive leadership experience across health systems and health plans, including the Chief Medical Officer of Cigna Medicaid (TX Plan). As a managed care leader, he developed deep expertise in medical management, care coordination, health benefit design, and cost of care management. Furthermore, he has spoken at local and national levels on clinical quality improvement, prior authorization, and provider-payer partnership. Currently, he serves as the director of utilization and oversees clinical care utilization, data analytics, and population health management for Central Health, a hospital district in Austin, TX. He completed his medical degree from Dhaka Medical College, in Dhaka, Bangladesh. He finished his training in internal medicine at Saint Vincent Hospital in Worcester, Massachusetts. His post-graduate training includes a fellowship in health services research from NYU (2003), and a Physician Executive MBA (2015) from University of Tennessee Knoxville. As a practicing general internist, he specializes in managing chronic conditions in adults, such as hypertension, diabetes, and cardiovascular diseases.

Dr. Lena Miller is a Managed Care Clinical Pharmacist and strategic leader with over 15 years of experience across Medicare, Medicaid, and Commercial pharmacy benefit programs. Her expertise spans formulary development, P&T operations, utilization management, and cost-containment strategy.
Working for a startup, Dr. Miller currently leads clinical strategy initiatives that combine real-world managed care insights with artificial intelligence to automate chart-note review, optimize site-of-care decisions, and reduce prior authorization turnaround time. Her work focuses on delivering measurable cost savings while improving efficiency and accuracy for payer organizations and clinical pharmacists.
James Walsh, MBA is a transformational healthcare IT executive with more than two decades of experience driving enterprise innovation, regulatory compliance, and digital modernization across payer, provider, and government sectors.
His career reflects a rare blend of technical acumen, strategic foresight, and operational execution, consistently delivering measurable improvements in patient care, organizational efficiency, and revenue growth. As Founder and Principal Consultant of James Walsh Consulting LLC, James advises Fortune 500 payers and hospital networks on Medicaid modernization, EHR selection, and cloud migration strategies. His leadership has reduced claims processing times by 30%, cut infrastructure costs by 25%, and guided hospitals through complex system transitions while ensuring compliance with HIPAA, HITECH, and CMS regulations. Previously, James served as Medicaid IT Strategy Director and Health Plan CIO at Cigna Healthcare, where he transitioned operations to a full Agile framework, improving accountability and accelerating delivery timelines. His oversight of UX/UI integrations and EDI transactions directly impacted Medicaid populations across Texas, enhancing access to care and strengthening compliance with CMS standards. Notably, he led Meaningful Use Stage 2 implementation across 84 hospitals, developed revenue cycle products generating over $100 million in revenue, and pioneered Agile adoption within enterprise IT teams.
James holds an MBA from the University of Phoenix and a BBA in Business Administration with Computer Science from Lamar University. He is formally trained in ITIL, Agile/Scrum methodologies, and holds a Six Sigma Green Belt. A U.S. Army veteran and former president of the 307th Bombardment Group Association, James brings discipline, vision, and a commitment to service into every leadership role. James continues to shape the future of healthcare IT by aligning technology strategy with organizational goals, empowering teams, and advancing patient-centered outcomes.
